We need a form for each of your children. The CHILD ONE form includes all necessary parent contact information. If you are registering more than one child, please click the consecutive buttons (Child Two, Three, etc.) which will not require you to re-enter contact information.

Adult volunteers who work with our Children and Youth Ministries, are required to complete the following documents:

• Volunteer Screening Form
• PA State Police Clearance
• Child Abuse Release & Clearance
• FBI Fingerprinting or Volunteer Affirmation
